Dog Boarding Waiver Form
This agreement applies to all services provided between Your Local Fur Mama, hereinafter referred to as "YLFM", and the pet owner(s) or the owners’ representative(s). All terms and conditions of this shall be binding on heirs, administrators, personal representatives and assigns of the owner and YLFM. This agreement shall be in force from the date signed and for all visits thereafter, or until another agreement is signed and dated.
Listed pet(s) are in good health and have not been ill or exposed to any communicable conditions within the past 30 days.
My dog(s) are current on all vaccinations for rabies, distemper, parvovirus, bordatella, canine influenza (when required), or any other vaccinations required by Your Local Fur Mama. All required vaccinations will be given to my pet at a minimum of 24 hours prior to check in. Should my dogs’ vaccinations not be current, a fee will be applied at time of pick up ($50). I understand that my unvaccinated dog(s) may be at higher risk of disease and that I will forfeit the ability to make any claims against, receive any refunds from, and will not be exempt from paying the required fee to Your Local Fur Mama resulting from failure to vaccinate my pets. I hold Your Local Fur Mama and its staff harmless for any death or illness occurring to my pet(s) resulting from failure to provide the listed vaccinations and will incur all expenses related to said death or illness of my pet(s).
Dogs arriving with fleas or ticks may be treated, at my expense, with appropriate bathing at the discretion of Your Local Fur Mama without any further notice or permission from me outside of this agreement. Dogs arriving with worms will be turned away or sent home without notice or permission from outside of this agreement.
It is Your Local Fur Mama policy that all dogs wear collars while on Your Local Fur Mama property. I will supply a collar for my dog(s) to be worn while at Your Local Fur Mama. If a collar for my dog(s) is not supplied, I agree to allow my dog(s) to wear a collar supplied by Your Local Fur Mama.
I further certify that my dog(s) has/have not shown aggressive or threatening behavior towards any person.
I am aware that if my pet(s) need to be provided dog food during their stay, that it is an additional charge at $5.00 per meal. All medication administration is $5.00 per day.
Your Local Fur Mama reserves the right to administer aid and/or use any available veterinarian for my pet should the need arise, whether Your Local Fur Mama can reach me or my emergency contact or not. I assume full financial responsibility for any veterinary and related expenses incurred, in addition to other fees for services provided at or by Your Local Fur Mama.
I am solely responsible for all harm and/or damage caused by my dog(s) to any and all persons, property and other animals while my dog(s) is/are on Your Local Fur Mama property whether my dog(s) is/are under my own control and care or under Your Local Fur Mama’s control and care. Your Local Fur Mama reserves the right to refuse services and/or admittance to any person or pet for any reason. Costs for repairs to property (beds, walls, water hoses, etc.) or from injury to people or animals will be my sole responsibility.
Your Local Fur Mama and their staff shall not be liable for any injury or damage to any person or persons, animals, or to any person’s property, during the care, boarding, daycare or grooming of my pet, regardless of cause. This includes negligence on the part of any other pet owner attending Your Local Fur Mama. I further agree that Your Local Fur Mama shall not be liable for any costs, attorney’s fees or expenses incurred because of my pet(s) participation at Your Local Fur Mama.
I agree that my pet(s) may be videotaped, photographed and/or recorded. Your Local Fur Mama shall retain exclusive rights to the results and all proceeds of tapings, photographs, and recordings.
I agree that Your Local Fur Mama is not responsible for any lost or damaged items. Therefore, I will not bring special or precious items to Your Local Fur Mama.
Cancellation Policy
A 24 hour notice is required for cancellations. If unable to cancel within 24 hours I understand I forfeit my deposit.
After Hours or Late Pick Up
To help expedite the check in process, all boarders need to be dropped off before 6:00pm.
There will be a half day charge for all boarding pets picked up after 1:00pm.
If I cannot pick up my pet during office hours, I agree to call Your Local Fur Mama and if staffing allows, request an “After Hours” pick up for a fee of $30.00 which will be added to my regular charges. If, for any reason, I am unable to pick up my pet as scheduled when reservation is made, I understand that Your Local Fur Mama will make every effort to accommodate keeping my pet and an additional $20.00 per day will be added to my bill.
Should I or my authorized representative fail to appear to claim my pet(s) or fail to contact Your Local Fur Mama after 48 hours of the scheduled pick up date, my pets will be considered abandoned and Your Local Fur Mama will become the legal owners of my pet(s) and are thereby authorized to rehome my pet(s) as necessary. Any expenses incurred for the transport, overnight care, feeding, veterinary care, medication, placement, adoption, etc. of said pet(s) shall be paid by me in addition to all charges incurred for services provided by, at or through Your Local Fur Mama.
Prices
The prices below are subject to change.
Dogs over one year: $45 per night
Dogs under one year: $55 per night
Holiday charge: additional $10 per night
Food provided: additional $5 per meal
Medication administer: additional $5 per day
Late pick up fee: half day charge
Your Local Fur Mama is not responsible for making sure your pet(s)' vaccines are current. We cannot accept your pet for boarding or daycare if we do not have up-to-date vaccine records. We reserve the right to turn away any pet(s) if records are not current.
By signing this contract, I am acknowledging that I have read, understand, and agree to its entirety.
I agree to pay for all Boarding and/or Daycare services and any other charges I have incurred as explained in this Client Agreement.
I agree to pay all accumulated charges to Your Local Fur Mama prior to the release of my pet or pets. Your Local Fur Mama reserves the right to not release my pet or pets until all charges are paid in full.
This Client Agreement will remain in effect from the signed date for all services scheduled, all services rendered and non-scheduled visits, and/or boarding. I understand that from time to time this Client Agreement may be modified and I agree to sign an updated contract as needed.
It is the responsibility of the Client to notify Your Local Fur Mama of any changes in client information, emergency information, pet profiles, or any other information important in the care and safety of the pet or pets attending Your Local Fur Mama.
By signing this Client Agreement electronically, all parties agrees that this Agreement and any other documents to be delivered in connection herewith may be electronically signed, and that any electronic signatures appearing on this Agreement or such other documents are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
